Ordered ttc girl plan

Hi! I'm really new and just ordered the customized ttc girl sway plan for $79. I know we're supposed to fill out a questionnaire first, but I'm so lost in how to find it. Is there any way someone could point me in the right direction? Thank you!

Thank you so much for your reply! I really apologize, but that link only gave me forms for the IVF portion. There were 2 firms that were ttc boy or girl, but it seemed like it was for if you had already conceived? I'm sorry again and I appreciate all the help!

Thank you so much! I'd like to get started right away and I know you're super busy so I'm completely okay with waiting however long for my plan! I really appreciate it. I just wanted to see if maybe pregnitude would be a good supplement to start? I've been diagnosed with pcos, but I'm not sure where I stand on that. I'm having irregular cycles so thought it might be an okay option to start?

I went over your questionnaire and my inclination is to try the alternate diet + exercise first but if you'd like to try it I'll put it into your plan. If we find weight flies off then we can always drop it.

Don't do cinnamon though. I think that would be too much of a good thing for us.
